Vsync OFF + FPS Limiter 200 (default value): Crashes.

Looks like I accidentally disabled Vsync, which made the crashes come back. I fixed it by letting Vsync disabled and setting FPS limiter to 144 (instead of 200, which is default).
2 crashes today in Giant's Shadow within an hour. Makes absolutely no sense I was able to play 40 hours without a single one.

Got one crash on Conquest Empire's Edge (Device Removed, 376.60) in 38hrs of playtime.
So not 100% fixed but it's rare enough to not bother me as much.

If your PC crashes with other games, it's likely an unstable overclock or overheat issue. Try to reset everything to stock settings (CPU, GPU, RAM) and see if it fixes anything, and check the temperatures.
