Pressing start, then down 2 on the d-pad will select the squad menu. That has fixed it for me every time.

Could it be players dual-boxing getting an advantage by being able to see where all the opposing players are located? You get somewhat of a Commander's view in the spawn screen. That and they could use it to switch to the winning team.

I do have it. The caveat may be that I only had 3 kills tracked when they changed the requirement.

Did you check the tanks in Your Company? I finished it a few hours ago and it unlocked and was added to my inventory.
